Peak Rehab was created in 2024 with the intention to provide onsite therapy services primarily in assisted living communities.  As people age, there are declines in strength, balance, and mobility which can lead to increased assistance from caregivers.  Peak Rehab implements skilled therapy services to improve function and prevent declines in mobility.

Timothy Herek

Occupational Therapist, Owner, Peak Rehab

Timothy Herek is a licensed Occupational Therapist and accomplished graduate of the Masters of Occupational Therapy Program at Saginaw Valley State University, earned in 2016. With over 8 years of hands-on experience, Tim specializes in providing compassionate, patient-centered care for elderly individuals in a variety of settings.  Throughout his career, he has worked in diverse environments, including hospitals, nursing homes, and outpatient clinics. Tim is passionate about helping his patients regain independence and improve their quality of life through personalized therapeutic interventions. With Peak Rehab in Michigan, his therapy approach is centered on understanding each patient’s unique needs and goals, ensuring they receive the most effective and empathetic care possible. 

Jeff Nievierowski

Physical Therapist, Owner, Peak Rehab

Jeff Nievierowski is a licensed Physical Therapist and proud graduate of Central Michigan University’s Master of Physical Therapy program (2005). With over 15 years of hands-on experience, Jeff has dedicated his career to helping older adults regain mobility, improve safety, and live more independently. His expertise spans outpatient care, home health, skilled nursing, and assisted living—making him uniquely equipped to understand and meet the complex needs of Michigan’s senior population. At Peak Therapy, Jeff brings a compassionate, results-driven approach to every patient interaction, ensuring that care is not only clinically sound, but deeply personal.

Sarah Calkins

Sarah Calkins is an accomplished healthcare professional with a Master of Science in Occupational Therapy from Saginaw Valley State University and a Bachelor's degree in Human Development and Communication Disorders from Central Michigan University.

With over 12 years of experience in direct patient care and management in skilled nursing facilities, Sarah has developed a robust skill set that bridges clinical expertise with operational leadership.

As the Director of Operations at Peak Rehab, Sarah plays a vital role in driving the strategic direction of the organization.

She leverages her extensive background in both patient care and management to ensure operational excellence and alignment with the organization's goals.

Sarah's leadership is instrumental in developing and implementing strategies that enhance patient outcomes, improve efficiencies, and contribute to the overall success of Peak Rehab.

Her unique blend of clinical knowledge, business acumen, and strategic vision continues to drive Peak Rehab's commitment to providing high-quality care and exceptional service.

General Manager/ Human Resource

Laura Keit

With over 20 years in customer service and more than a decade in medical administration, Laura brings experience, warmth, and efficiency to everything she does. She holds an Associate’s degree in Applied Sciences – Business Administration, with a focus on Medical Office Administration.

Laura specializes in insurance verifications, prior authorizations, onboarding new therapists, and credentialing providers with in-network insurances.

Director of Rehabilitation- Northern Region

Emily Leivo

Emily Leivo serves as Director of Rehabilitation at Peak Rehab, where she leads  high‑ quality,  patient‑centered rehabilitation  services with a clear focus on outcomes, innovation, and compassionate  care.  With her blend  of advanced  clinical  expertise and executive‑ level  healthcare  leadership. 

Emily has over 18 years of experience as a  Physical Therapist Assistant (PTA)  since  graduating from Delta College. She is currently  the only PTA in  Michigan with  advanced proficiency in neurology certification from the American Physical Therapy  Association—a distinction that  reflects  her advanced  expertise in treating  complex  neurological conditions.  She also holds a master’s degree  in  healthcare administration from the  University of Phoenix. 

Emily’s commitment to healthcare extends beyond the clinic walls. She is actively involved  in community health  initiatives,  including serving on the  Community Advisory  Council for local school-based  health clinics, volunteering  with local  school athletic  programs, and serving as  an elected trustee  demonstrating  her belief that  rehabilitation and wellness  begin  within the  community. She is also  a member of both the American Physical  Therapy Association and American College  of  Healthcare  Executives.
